Let's say someone has 200 magical def.

Is it converted into some other number by some kind of ratio, and that number is subtracted from each hit, or is it converted into a percentage, which is subtracted from each hit?

And how do penetrations work? Does 45 pen simply give you the ability to ignore 45 def, or is there something more complicated going on?

This is how it works

Your damage : ((100+enemy's protection-penetration):100) = actual damage

Meanwhile here how penetration works

Your protection - aura penetration - percentage penetration - raw penetration = actual protection

Penetration can work in two different ways depending on what item it is. Items such as Voidblade and Jotunn's Wrath is known as flat penetration, which lets you ignore the protection of your enemies by the same amount. For example Jotunn's Wrath ignores 5/10/15 Physical Protection.

Item such as Titan's Bane on the other hand works in a different way, as it takes your enemies total protection value and lets you ignore a percentage out of it instead of just flat penetration. For example an enemy with 100 protection against Titan's Bane would let your attacks and abilities ignore 33 protection done to that target, while enemies with a lower amount, let's say 60, would only let you ignore 20 protection.

Formula for Titan's Bane: (enemyProtection / 3) / 100 = protectionIgnored

Formula for The Executioner: ((enemyProtection * 0.79) - 15) / 100 = protectionIgnored

When it comes to Protection it works in a way that for each point of protection you have, it increases your effective health by 1%. For example when you have 100 Magical Protection you'll have effective health equal to your total health multiplied with 1/0.5 against magical damage, so if you have 2000 Health and 100 Magical Protection, it will increase your effective health against magical damage by 100% of your total health, giving you a effective total health of 4000 Health against magical damage.

So in your question about the 200 Magical Protection, you would have an additional health equal to 300% (33 goes 3 times in 100) of your total health against magical damage, and if you have 300 Magical Protection it would be additional health equal to 400% (25 goes 4 times in 100) of your total health against magical damage.

Formula: totalProtection / ((totalProtection + 100) / 100) = %damageMitigated

Formula: totalHealth * (1 / (%damageMitigated / 100)) = effectiveHealth

I couldn't follow your numbers.

If you have 100 protection, then damage is mitigated 100 / (100 + 100) = 50%, right? Assuming of course the opponent has no penetration.

So if you are taking half damage (using zero protection as our baseline), wouldn't your effective health be essentially double your actual health at 100? So 4000 [2000 * 1/.50 = 4000], not 3000.
