Pestilence is also a good option if the healing is magical, or if they simply have a lot of magical damage and physical healing + magical healing.
This item is dependent on whatever is causing you the most trouble.
For example, if the threat of magical/physical is equal - go Mantle of Discord/Hide of the Urchin/Spirit Robe/Shifter's Shield depending on what you prefer.
If the threat is physical then: Hide of the Nemean Lion/Breastplate of Valor/Void Shield.
If the main threat is magical then: Pestilence/Bulwark of Hope/Genji's Guard.
Blink is essential for early-game ganks, and Aegis/Puri Beads are situational depending on if the enemy team has high dmg ults (aegis) or a lot of CC (Puri Beads).

Basic Attacks and Abilities
This build makes the most of Thanatos' kit as it utilises his immensely strong abilities, whilst also his very strong basic attack sequence. His first two basic attacks hit single targets, and the final attack acts as a cleave, hitting everything in front of him.
Passive:
- This passive ensures that Thanatos heals after every enemy and minion he kills, and is crucial to his sustain. Once this passive is stacked with
and
, it can become almost impossible for you to be killed - so long as you utilise your kit correctly.
Ability 1:
- This is your most important ability during the early-game, and can save your life late-game. During the early game you must ensure you hit this ability if you are to secure a kill, as it can prevent you dying and kill an enemy in the blink of an eye. Late-game, it can act as a small lifesteal that can sustain you in fights.
Ability 2:
- Another crucial ability, this increases your speed when travelling towards enemies within the threshold of your ultimate. This speed reaches an insane value at high level, and can make your enemies' escape almost impossible. Further, it increases your penetration significantly, so you must ensure it is constantly active during fights.
Ability 3:
- This can save you and your allies' life, as it is a damaging silence that can interrupt many abilities as well as prevent them. Use it wisely during fights and prioritise it on enemies with high damaging abilities e.g. mages.
Ability 1:
- This is essentially an execution ability that heals you significantly, however it can also be used to initiate combat as well as escape. Focus your executes on the targets you struggle to kill the most, and make sure you do not get tricked into activating it early - if you know they have mobility, wait for them to use it and then execute them as punishment.
Early Game
In the early game, Thanatos does not need to use abilities on XP camps, as his basic attacks can return him to full HP due to his passive - simply aim for the central, larger minion and attack. For Mana/Speed etc., use your
on the central minion and carry on with basic attacks.
For early ganks, using Blink and your
is immensely strong, and once you have your
it is essential that you use this as early as possible to silence and prevent escape.
Late Game
By this point, you should be fed and your enemies will know what to expect. Since you are relying on your basic attacks once your abilities have been used, you need to practice quickly snapping your basic attacks onto nearby enemies to ensure every hit counts, and that you get the healing you need in order to sustain the fight.
Your ultimate can be crucial late game, as their tankiest player can be taken out by simply diving down on them whilst they sit at 40% HP. Once this player is out of the picture, the most vulnerable and highest damaging players must be taken out, as they are your biggest threat. Examples are mages like

I will give you credit for building two crit items instead of just Deathbringer, but I agree with the others that Than doesn't do well with a crit build.
